The Forms Committee Chair is responsible for assisting the Band Director in assembling,  distributing and collecting required band forms and maintaining the CHARMS band database.  Should be comfortable working with a spreadsheet and performing data entry. The activities for  this position occur primarily in the spring, to a much lesser extent in the fall and minimally  throughout the year.  

• Assembles Band Packets for all students taking band the next calendar year (including rising ninth  graders).  

• Enters student information into the CHARMS band database.  

• Files returned forms.  

• Assists with the collection of required forms that are not turned in by deadlines or are incomplete.  Running periodic reports for use by band board/committee chairs and for auditing database.  Assist in the development of data entry guidelines for the CHARMS band database.
