Both reimbursement forms are now electronic forms.  You may fill them out on your computer and email them back to the Treasurer.

• Expense Reimbursement Form

To obtain BPA reimbursement for an expenditure made on behalf of the band, the BPA Expense Reimbursement Form must be submitted to the board.  If the expenditure is $25 or more, the Expense Approval Form, signed by an Executive Board Member, must also be submitted.  Expenditures under $25 must be approved by a BPA Board or Committee chair; however, no formal documentation of this approval is required.
